Hospital Fast Pass Registration

If you are scheduled for a surgery or procedure at our hospital, follow these steps:

Step 1: Preregister

Preregister online prior to your procedure by completing the preadmission form.

Step 2: Upload Documents

Upload a scanned copy or photo of your current driver’s license and insurance card using the form below.

Each downloadable document below must be signed and uploaded to complete preregistration. If you are unable to fill out the forms, call (225) 763-4777 and someone will assist you.
